An entirely respectable choice as a personal favourite. :)

To address a few of your points:

1) I thought the failed hit on Pentangeli was set up by Roth? At least, I always assumed as much.

2) I'm not sure when Michael realised Roth tried to kill him either.

3) The brothel scene : I actually love how ambiguous it is. We are left to decide what really happened, which allows for some seriously dark interpretations. In a way, it is akin to the "horse's head" moment from the first film, but with more ambiguity. I should have said that in my review really.

4) The Cuba section is masterful. I didn't realise the gold telephone/parking metre and slot machine details were based on real events. Thank you for pointing that out.

5) The Fanucci hit sequence is astonishingly vivid, I agree.

Thank you for reading and commenting. :)
