I am sure it did go over the heads of some US viewers. Kubrick's genius was to treat the Cold War, rightly, as material for farce. It was absolute lunacy and nuclear brinksmanship remains absolute lunacy. Because it is simultaneously stupid and horrifying, it became the ideal satirical subject and ideal for pitch black comedy. :)
