Since mystery is a major part of my storytelling ouevre, this is the key component for me. I've only written one novel with a prologue (this one) but I believe it fulfils your criteria and is vital to the narrative.

Great article, by the way. Prologue-phobia annoys me, because they are like any other literary tool, and are either done well or badly. :)
